Appointment booking field type

When setting up a Formidable Form, you can add an Appointment Booking field to the form. Pre-select an appointment type or let your customer select what type of appointment they’d like to book.

As customers complete your form, they optionally select an appointment type, then a date and time for their appointment.

Once an appointment is booked, you can access the appointment in Simply Schedule Appointments from the form entry.

Or, when viewing the appointment in Simply Schedule Appointments, you can access the Formidable Form entry.
